Poland’s Syndrome - Patients treated with the Adjustable Gel Implant

Poland’s Syndrome consists of a deficiency of tissues on one side of the upper body. It may include underdevelopment of the breast, chest muscles or hands and fingers. Severity can range from mild such as breast asymmetry to severe where the muscles and arms are involved.

Symptoms

1. Absents of part or all of the pectoral muscles and part of the underlying bony structure.
2. Under development or absence of the breast and nipple on the affected side
3. Under developed arms, hands and fingers

Poland’s Syndrome may be associated with other syndromes such as Moebius and Klippel-Fiel syndrome.

Plastic surgeons are commonly involved with treatment of the breast deformity. The newly developed Adjustable Gel implants offer a new treatment option in the treatment of Poland’s Syndrome Breast deformities. They are used to stretch the underdeveloped tissue and obtain symmetry.

When muscles are missing the posterior muscles of the back can be rotated to the front. Another new option is the use of acellular dermal grafts

Chest wall deformities may require thoracic surgery and hand deformities are usually treated by a hand surgeon.

Fortunately, now that Silicone Gel Implants have been approved by the FDA, they offer a viable solution to many of the problems that were caused by saline implants.

“Our patients who have had problems with saline implants are delighted by the feel and shape of the new gel implants. By replacing gel implants above the muscle, abnormal movement is eliminated and a more natural shape and feel is restored.”

BREAST RECONSTRUCTION FOR PATIENTS WITH POLAND’S SYNDROME

Treatment options include:

Customized chest wall implants.

Tissue expanders which are then subsequently replaced with breast implants and now recently developed adjustable gel breast implant which functions as a tissue expander and a breast implant.

When a pectoralis major muscle is absent it is reconstructed using a flap from the back.

Autologous tissue, that is tissue moved from one part of the body to another in the form of a pedicle flap that is attached to the muscle or is a free flap where it is replanted micro surgically.

Recently, acellular dermal grafts have been used to replace missing muscle.
